Start Treatment in 3-Easy Steps

1. Intake and Labs

Set an appointment through the “ Book an appointmen t” link. Select your state and choose a time with the provider in your state under “ new patien t”. Complete an online medical form for your comprehensive health panel sent to your email . Lab order will be generated, and a staff member will call to confirm and answer any questions.

2. Visit Labcorp Diagnostics

Walk-in to any LabCorp Diagnostic facility where they will do a quick lab draw, your Habitat Healthcare provider will receive the results within 48-72 hours.

3. See Your New Provider

Receive a Video or Phone call from your doctor and if approved you will receive your medication in the mail.
